Overview
Adstem 24mg Injection contains plerixafor, a stem-cell mobilization medicine used before autologous hematopoietic stem-cell collection. It does not treat the underlying cancer directly. Plerixafor Injection works by blocking CXCR4, disrupting the signal that retains stem cells in bone marrow and allowing CD34+ cells to enter the bloodstream for apheresis. Adstem 24mg is used with G-CSF in patients with multiple myeloma or non-Hodgkin lymphoma being prepared for autologous transplantation. The timing of plerixafor 24mg, G-CSF, and stem-cell collection is important for effective mobilization.

Uses and Indications
In combination with G-CSF, plerixafor 24mg injection promotes the migration of hematopoietic stem cells from the bone marrow into the peripheral blood, making it possible to harvest the stem cells for autologous transplantation.
Adstem 24mg Injectionis primarily indicated for use in adult patients undergoing preparation for transplantation, especially in patients for whom stem cell mobilization may prove problematic.
- Multiple Myeloma: For the harvesting of hematopoietic stem cells prior to autologous transplantation.
- Non-Hodgkin Lymphoma: In combination with G-CSF for improved mobilization and harvest of hematopoietic stem cells.
- Poor Mobilization: Useful in patients anticipated to or who have previously had problems collecting a sufficient number of hematopoietic stem cells
Plerixafor Injection is not usually recommended in stem cell mobilization in leukemia patients because it could result in the mobilization of malignant cells in addition to hematopoietic stem cells.
How Adstem 24mg Injection Works
Adstem 24mg Injection is able to reversibly inhibit the action of the CXCR4-CXCL12 signalling system that plays an important role in retaining CD34+ stem cells within the bone marrow niche. Inhibition of the retention process leads to migration of more stem cells to the bloodstream.
Clinical Evidence for Adstem 24mg Injection
Phase III NHL study: Plerixafor 24mg combined with G-CSF helped more patients collect the required number of CD34+ stem cells compared with G-CSF alone.
Phase III myeloma study: Plerixafor injection plus G-CSF improved stem-cell mobilization and collection, helping more patients reach the required stem-cell target.
Side Effects of Adstem 24mg Injection
Just like any other medication, Adstem Injection is also associated with some side effects. Most of the side effects are minor and usually go away within some time.
Common side effects
- Diarrhea
- Nausea
- Injection-site reactions
- Fatigue
- Headache
- Dizziness
- Abdominal discomfort
Serious side effects
- Anaphylaxis
- Splenic enlargement/rupture
- Leukocytosis
- Potential malignant-cell mobilization in leukemia
Critical Safety Warning
TRANSPLANT-TIMING WARNING: Timely administration of Adstem 24 mg injection is critical since it should be administered in an appropriate way along with G-CSF and stem-cell collection (apheresis). It has been reported to increase the size of the spleen in some patients and may even lead to splenic rupture. Pain in the upper-left side of the abdomen and pain in the tip of the left shoulder should immediately be attended to by a doctor.
Adstem 24mg Injection Dosage and How To Take It
Clinical context | Typical labelled / protocol guidance |
Usual adult dose | 0.24 mg/kg, given under the skin once daily before apheresis, as directed by the transplant team. |
Renal impairment | If kidney function is reduced and CrCl is 50 mL/min or less, the dose is commonly reduced to 0.16 mg/kg. |
Combination | Adstem 24mg injection is given after G-CSF has been used to mobilize stem cells. The transplant team decides the exact timing. |
Maximum dose | The daily dose should not exceed the maximum amount allowed in the current medicine label. |
Important: This is just a general guideline for your understanding. Your healthcare professional will determine the correct dose and frequency and combination with G-CSF and make changes according to your kidney functions, responses, and side effects.
Adstem 24mg Injection Missed Dose
If you miss an Adstem 24mg dose, it will impact the schedule of stem cell harvesting. Please call the transplant team right away; however, you must not take any other dose of the medication yourself.
Safety Advice for Adstem 24mg Injection
| Category | Status | Guidance |
|---|---|---|
| Pregnancy | Avoid | Inform the doctor if pregnant or planning pregnancy before receiving Adstem 24mg Injection as this may affect the health of unborn baby. |
| Kidney | Dose adjustment | The doctor should check kidney function and adjust the dose of Plerixafor 24mg Injection if needed. |
| Spleen | Urgent review | Report severe pain in the upper-left abdomen or left shoulder immediately to ensure safe and effective treatment. |
| Driving | Caution | Plerixafor 24mg may cause dizziness, however, avoid driving or operating machinery during the treatment. |
Monitoring During Adstem 24mg Injection Treatment
- Blood levels of CD34+ stem cells will be monitored, and if there is no appropriate rise, the treatment plan will be modified accordingly.
- CBC will be done for monitoring red blood cells, white blood cells, and platelets.
- The function of the kidneys will be tested using blood tests, and your treatment will be assessed in case of any changes.
- Pain or feeling of being full in the upper left abdomen or unexpected pain in the left shoulder must be brought to your doctor’s notice right away.

FAQs About Adstem 24mg Injection
Does Adstem 24mg Injection treat cancer?
No, Adstem 24mg Injection is not used to treat cancer itself. It is used to mobilize and harvest stem cells from blood for autologous stem cell transplantation. These harvested stem cells can then be used as part of the treatment for cancer or other blood disorders.
Why give G-CSF too while using Plerixafor Injection?
G-CSF is administered with Plerixafor 24mg injection since the combination promotes the transfer of more stem cells from the bone marrow into the bloodstream, making it easy to collect enough stem cells for autologous stem cell transplantation.
Why is timing important?
The timing of when you take the medicine is important since the level of stem cells should be high just before the apheresis process. However, the correct timing ensures that you have adequate stem cells in your blood.
Does kidney disease change the dose of Adstem 24mg Injection?
Yes. In case of decreased kidney function, an adjustment in the dose of Adstem 24mg injection may be necessary since the drug is excreted via the kidneys. The physician will assess kidney function and give the proper dosage according to the degree of dysfunction.
Can Plerixafor 24mg Injection be used in leukaemia?
Normally, Plerixafor 24mg is not prescribed for stem cell mobilization in patients with leukemia. The drug can also cause the mobilization of cancerous blood cells, along with the normal stem cells. Thus, the decision on the use of the drug should be taken on a case-to-case basis by the doctor.
How is Adstem 24mg given to the patient?
Adstem 24 mg Injection is taken subcutaneously, and the common site for the administration of this drug is the abdomen. Hence, this drug is usually prescribed or recommended by the doctor itself.
What symptoms are urgent during the treatment?
Severe allergic responses, such as shortness of breath, swelling, or even loss of strength, demand immediate medical care. Any kind of pain or fullness of the upper left part of the abdomen or any sort of pain in the left shoulder while taking Adstem 24 mg should be immediately reported to the doctor
